• Sign and date the form.
• The form will need to be witnessed by a notary public and stamped.
• Mail the form to the Problem Gambling Coalition of Colorado.

What Will Happen After I Sign Up For A Self-Exclusion List?

Once you are signed up for the program, you cannot access gambling services.

Casino gaming and online services are void.

Your personal information, such as your driver’s license and description, are added to a database.

If you tried to sign up for an online account, the information you provided would be flagged.
